What are the benefits of online services
Online services will allow you to complete any of the below at a time that is convenient to you – day or night, seven days a week. All patients have the opportunity to use Online Services to be able to:
- Order repeat prescriptions
- Change or amend personal details
- View your summary care record
- View your detailed coded record
Online access helps to manage appointments and telephone calls more efficiently and effectively, and patients find it convenient and accessible.
How to register for SystmOnline
To register for our online services, you will be required to complete a registration form. You may download and print your own registration form by clicking here, or ask for a form at reception.
Please bring the completed form, together with your photo ID and address verification documents to the surgery. The service is available for patients over 12 years of age.
For patients under the age of 12, access to online services can be granted via a parent or guardian’s SystmOnline account, provided that relevant ID is seen.
Once your application has been processed, you will be sent your log in details and password. This will be sent to your home address, so please ensure we have the correct address for you.
Your log in details may be sent via email or text messge if this is your preferred method of contact. Once you receive your log in details, you will then be able to access the Online Services. You may change your password at your own convenience.
Patient and Practice Responsibility
Montague Medical Practice has the right to remove online access to services for anyone that doesn’t use it responsibly.
It will be your responsibility to keep your login details and password safe and secure. If you know or suspect that your record has been accessed by someone that you have not agreed should see it, then you should change your password immediately.
If you are unable do this for any reason, we recommend that you contact the surgery so that we can remove online access until you are able to reset your password.
If you print out any information from your record, it is also your responsibility to keep this secure. If you are at all worried about keeping printed copies safe, we recommend that you do not make copies at all.
